What you need before admission

Insurance

The very first thing you would need to do before discussing admission with your doctor is to find out from your insurance company whether the upcoming procedure is covered by your plan. We are direct billing partner with a number of insurance companies; if you are their client, we can file claim on your behalf. If we are not directly billing partner with your insurance company, admission cost is in principle all out-of-pocket and paid-in-advance. (Go to Payment and Insurance for more information.)

 

Emergency Admission

We will do our best to ease your emergency admission worries as best we can. Our staff is very experienced working with insurance companies, we might ask you or your family or friend for insurance information, and if you are Tricare member, we might even ask for your social security number.

Admission Papers

Once your admission date is set, you will have a chance to speak to our nurse and administrative staff to address any questions or concerns, and to also receive the necessary admission papers to take home to read and sign.

  • Procedure-related consent form(s)
  • Letter of Commitment for Hospitalization
  • Medical payment agreement
  • Consent form for use of hospital gown
  • Consent form for use of adult absorbent products (depends and/or absorbent sheets)

Necessary Items from Home

Having the following items from home will make your stay a bit more pleasant.

  • Toothbrush, toothpaste, shampoo, soap, etc.
  • Your favorite drinking cup, silverware/ chopsticks
  • Your favorite book or movie
  • A box of tissue (some people even bring their own toilet papers because Japanese toilet papers are rather rough)
  • Face towel or bath towel
  • Clean underwear
  • Pair of slippers
  • Laundry bag (if necessary)
