Payment and Insurance

Payment Policy

If you do not have Japanese public health insurance or any of the partner insurances listed below on this page, you are responsible for making full payment in cash or by credit card.

For patients without Japanese public health insurance, medical charges are calculated at 20 Yen per medical point.

We accept American Express, Master, Visa, and JCB.

Japanese Health Insurance

If you have Japanese health insurance, please present your residence card and insurance card at the time of check-in.

Traveler Insurance

If you have traveler insurance, your medical bill will be out-of pocket and we will supply you the necessary documents for you to file claim on your own.

U.S. Health Insurance Partnership

We are direct billing partner with the following insurance companies. PLEASE CONTACT US about obtaining a Guarantee of Payment/Benefit before making an appointment.

TRICARE, aetna, BlueCrossBlueShield, afspa, Cigna

Please have your insurance card with you at least for the first visit.

ALL Guarantee of Payment/Guarantee of Benefits must be issued directly to the e-mail address above at least a day before service.

● TRICARE PRIME

• Direct billing for outpatient, inpatient, and emergency

If you are active duty or family member, please see your doctor on base first for a referral letter, and wait for Tricare to contact you for an appointment offer.

● TRICARE SELECT or TRICARE FOR LIFE

• Direct billing for inpatient only

If you are a veteran, we advise you see your doctor on base first for a referral letter to be eligible for subsequent medical supports on base.

All outpatient visits are full out-of-pocket payment. You would need to file claims on your own for reimbursement. If you also have Japanese health insurance, use that as your primary health insurance.

We can file claim for your inpatient care. However, you are responsible for your catastrophic cap of 35,000 Yen at time of discharge.

● AETNA at EURO-CENTER

• Direct billing for outpatient, inpatient, and emergency

If you’d like us to file claim for you, please contact Euro-Center at the following number and ask for a “guarantee of payment” issued directly to us least a day before your scheduled appointment. You will need a new GOP PRIOR to every single outpatient visit and admission.

● BlueCrossBlueShield Federal Employee Program

• Direct billing for outpatient, inpatient, and emergency

If you’d like us to file claim for you, please contact GMMI at the following number and ask for a “guarantee of benefits” issued directly to us at least a day before your scheduled appointment. You will need a new GOB PRIOR to every single outpatient visit and admission.

● afspa – Foreign Service Benefit Plan

• Direct billing for outpatient, inpatient, emergency, and annual physical

If you’d like us to file claim for you, please contact afspa and ask for a “guarantee of benefits” issued directly to us at least a day before your scheduled appointment. You will need a new GOB PRIOR to every single outpatient visit and admission. You will also need to complete a claim form each time. You can fill out the form ahead of time (leave the diagnosis blank), or fill it out during your visit. (Download claim form)

You are also covered 100% under this insurance for routine physical at our facility. Please check the preventive medicine page and see which package you are covered.

● CIGNA

• Direct billing for outpatient, inpatient, and emergency

No guarantee of payment is needed for outpatient services below 400 USD (or approximately 45,000 Yen). Please contact us first for estimate of your first visit.

You will need a GOB PRIOR to every inpatient admission. You will also need to sign a claim form for each visit.

Please note:

Please make sure that we receive your GOB/GOP at least a day before your visit. Without such, you will be responsible for the total medical expense. We ask that you make your co-share/deductible payment in cash if possible, in case we have refunds for you. Thank you for your cooperation.
